The Exchange Group will also announce internal penalties for its CEO Kiyota Akira, TSE President and CEO Miyahara Koichiro, and other top executives.
Miyahara "has requested to resign from his position as President & CEO of TSE and all other held positions within the Group, effective on November 30", it said.
Financial regulators last month conducted an on-site inspection on the exchange to investigate the causes behind the outage. At a news conference on Monday afternoon, Kiyota apologized for the system crash.
The Oct. 1 outage came as Japan has been pushing to boost Tokyo's standing as a global financial centre.
The latest shutdown was caused by a setting error, which prevented an automatic backup from activating after a memory failure in the TSE's trading system.

He said the company was not thinking of seeking damages from Fujitsu, but said the system developer should also feel "significant responsibility".
"The all-day trading halt at Tokyo bourse significantly undermines investors' trust", the Financial Services Agency said in a statement, noting the exchange needs to clarify where responsibility lies.
The all-day trading halt was the worst-ever outage since the world's third-largest equity market switched to all-electronic trading in 1999. The exchange has said the glitch was the result of a hardware problem at the "Arrowhead" trading system and a subsequent failure to switch to a back-up.
The TSE has said a committee was due to draw up fresh guidelines by March on how to restart trading following a system failure. Only the headline has been changed.
